JVN Awards 2025 - Lifetime Achievement Award Submission Form


This award recognises volunteers who have made an outstanding, lasting contribution to their charity and the community. This year, every Lifetime Achievement submission will be celebrated – with every charity winner invited to a special event in Hendon on Thursday 6th November, and their stories shared at the Awards and across our platforms.

We invite you to put forward your Lifetime Achievement nominee and be part of this unique celebration of those who have dedicated so much to our community.

This is a reminder: please make sure you sign in to your JVN account before filling out the submission form; otherwise, you will not be able to save your form as a draft.

Once you have completed the form, please make sure you make the relevant payment. You can pay for the submission here. Please note that you should only access the payment page when all the submissions for your charity are complete. 

Please include your charity's website so that the judges can understand a bit more about your charity.

Please take the time to write as much detail as possible about your Charity Winner and explain what makes them stand out. This information along with the supporting evidence is all our judges will have access to for their review. 

You will be asked to submit some personal details about your Charity Winner in this form. These details will not be passed on to any third parties. Please check this box to confirm you have permission to share them with JVN.

This will be the name which will be put on the trophy

  • What makes your volunteer perfect for the Lifetime Achievement Award? This can include but not limited to:
  • Length of service
  • Role development over time
  • Uniqueness bought to the role

Please provide specific examples.

How has your volunteer been inspirational to those involved in the charity?

Possible examples can include:
- Overcome challenges
- Helped other volunteers
- Taught new people within your charity what to do

Please provide specific examples.

How has your volunteer shaped or developed the charity itself?

Possible examples:
-Created a new project within the charity which has caused the charity to further grow
-Developed projects to help more service users
-Improved one of your charity’s processes

Please provide specific examples.

How do you believe your volunteer has shaped the Jewish Community and/ or the voluntary sector as a whole?

Has your volunteer consistently supported both your charity and volunteer team, by living the values of your charity?

This two-line summary should highlight why your Charity Winner has been selected. This summary will be used in our brochure. 

Please make sure the photo meets the requirements outlined below: 

- Your charity winner is alone in the photo.
- The photo is of high quality and has a good resolution 
- The photo has space around your charity winner's face, do not crop a photo too closely. 

If you do not have a photo of your charity winner, this can be sent separately via email. Please upload an image of your charity's logo so you can submit. 

If appropriate, please upload up to three relevant attachments containing supporting evidence or references along with your application. Please only use this option if you feel it will enhance the nomination. Please note that only Word, PDF, JPEG and PNG files are accepted. You may upload your supporting evidence here.
